To give a serious answer to your question they might have been to areas like Western Sahara which is disputed. Or gone to South Sudan before it was independent and doesn’t know whether to count it. These are two examples. I have been to Aruba which isn’t a country but it’s administered by the Netherlands but I haven’t been to the Netherlands so that’s kind of the same.

how long do you stay in countries? do you consider that some are a LOT bigger than other and thus should need more time to visit in a satisfactory way? i.e. India has more people than north and south america combined. or brazil is nearly as big as europe sizewise.

most people who count countries have a warped view of the world in my opinion!
