I had this issue a while back. In general it’s not the law it’s just the hospital policy. Might be different (not sure) if you are refusing medical attention.

I've been in hospital a couple times and don't recall having to sign anything at discharge.

If you are stealthy or just run fast you don't have to sign anything ever

No, but if you leave AMA (against medical advice) insurance may not pay

They told us this, but we’ve looked into it and it is a commonly believed MYTH! Look it up, there’s zero truth behind it.

Very interesting! I just did a quick search, there have been studies done and no cases have been found of insurance refusing to pay. While that doesn't necessarily mean they can't refuse payment, not being able to find any cases where it has happened strongly supports the idea that it can't happen because we all know insurance companies will use any means necessary to deny payment. I suppose we would have to read the insurance contract we signed with them to see if they "reserve that right". Logically, I would think they would want to refuse to pay for any action that increases the likelihood that you will get sicker (and use more of their money).
