Since armor and the associated garments were almost all custom-made, I've often wondered how many medieval armorers and tailors were actually creating orthopedic devices through trial and error.

Exactly my thinking. They also probably had extremely blunt patient/customer feedback to aid in their development process.

A lot of it was survival of the fittest with a healthy dose of "holly shit that almost killed me i need armored armpits."
