It's not about shouting, it's about cramming as much profanities into a unit of time as possible. You see, now I can call your mother a cow with my mouth while I tell you to fuck yourself with my fingers.
Actually, hand gestures act in langage the way emoji acts in writing. They accentuate what is said, but can also convey a proper meaning that would ADD to the speech.
So, yes, in a public space with lots of noise, hand gestures are helpful.
But you are right that OP's comment on Italians as a whole made me grind my teeth.
