Aren't caffeine and nicotine both vasoconstrictors? How would the combination be a vasodilator?

It’s definitely a vasoconstrictor. I have arteriovenous malformations that ache when I take vasodilators (including alcohol). Caffeine reduces pain from the malformations due to being a vasoconstrictor. I can feel it working.
