This is why health care is inherently incompatible with free market capitalism. I cannot choose to boycott a provider; rather, I am held hostage and forced to spend money at the provider's discretion. If I need a refill on a medication, and the provider forces me to come in for an in-person visit with an out-of-pocket cost prior to refilling it, there is no recourse. I can either pay the provider what they are forcing me to pay, or I can start over with a new provider and pay them more.

That's why pure free market capitalism is a bad idea. Some things cannot be sustained on a for-profit basis if they're meant to be accessible to everyone, healthcare, unemployment insurance and education for example. These must be tax funded and not operated with profit in mind.
