Report to 311 either homeless outreach or NYC WELL so an outreach coordinator can connect with her to start to get her help if she is is mental health crisis or a schizophrenic episode.

Is it possible to report someone like her because I don't know where she is located exactly and it looks she's still on the streets.

Yes. 311 allows for general location descriptions. Could also report via 311 Twitter via DM. That format allows for more flexibility in reporting.
