Probably nothing, to me this looks very similar to high frequency vibration damage which can happen if you mount your cell phone to a motorcycle or some other engine.

I'm not saying that's what happened though but this looks like a damaged camera struggling to focus correctly. At that point there's not much you can do outside of replacing the camera in the phone.

Other people have reported issues about a lot of glare showing in the photos. Seems like a defect and this might be related. I’d take it to the Apple Store. They’ll probably replace it under warranty.

Do you have a screen protector? Maybe take it off and clean the actual screen and then apply a different one that doesn’t cover the lenses

Which model? I’ve had a 14PM do this as well. Plenty of discussions with Apple support over their app chat feature. On a replacement after the camera grinding so it isn’t leftover damage from that issue either. All the troubleshooting and diagnostics show “no issue” but I do get questioned about my MagSafe OtterBox case and if I’ve had it mounted to a motorcycle or bike (have not). In person I’ve talked to att store and apple authorized Best Buy who looked at the phone and then my blurry photos and are also confused. Best advice I got was from Att store to keep making contact with apple using the chat exclusively so you have a record. It is easy to do from the support app.

Dirty lens would be my first guess. Second would be that maybe you’ve inadvertently lowered your photo quality in settings - definitely check that. Failing that then yeah it’s probably some sort of issue with the phone.

Check for steam or moisture inside the camera under the camera lens glass. Also as other have said , make sure the camera lens is clean you can wipe it with alcohol and a microfiber cloth

I didn’t have any issues with the selfie quality In the past but now I’ve noticed my selfies turned out garbage when in daylight but turned out good when in good lighting
